This CAF can be used for:

a) New Connection

b) Address change ( Name / Ownership change not allowed )

It is mandatory for every new service connection request to be submitted with a unique CAF number. Multiple connections can be subscribed on a single Unique CAF. Pl avoid taking photocopy of the CAF image downloaded for submitting service requests on different names.
